Workflows are used to create a multi-step approval process for a form submission. To create a Workflow, go to the Dashboard, select Surveys & Forms, and select Workflows. Select Create a Workflow in the top right.
Name the workflow and click Save.
Once you have created the workflow, it is important to connect the workflow to the form you are using. To do this, navigate to the form and select Edit.

Select the settings menu in the top left of the form, and under the Basic Information tab, navigate to Select a workflow template and select the workflow you would like attached. Select Save.

Now that you have connected the form and workflow, you can edit your workflow and create the multi-step approval process. To do this, navigate to the Workflow and click Create Step in the top right (or edit the default steps that are generated when the workflow was created).
Provide the step with a Name. Then, set the transition - this determines how many people need to approve in order to transition to the next step. You’ll also need to set if this step is activated by default. If this is set to Yes, all submissions will receive this step; if this is set to No, submissions will only receive this step if a logic in the form invokes this step to happen. Click Save when you have configured the settings.

If you set Activate by default to No, navigate to the form that is attached to the workflow and go to the question that should invoke the logic. Click on the Logics button for that question.

Set the Logic Type to Workflow, set up the appropriate question logic, set the logic to then Add to the Workflow, and select the appropriate step that it should invoke when the condition is met. Click Save when done.

Return to your workflow to continue editing the step. Click on Edit Teams to choose what Teams are part of that approval step.
Select Create Team if you need to create a new Team.

Give the Team a name and choose how team members will be added to the Team.
Select Edit Participants to choose who is on the Team. Select OK when you have added members to the Team.

Continue to configure as many steps and as many Teams as needed to complete the workflow.
Once the workflow is configured, a submission to the form will kick start the workflow’s first step.
